POSITION SUMMARY:

Responsible for coordinating the development of operational and administrative infrastructure for chronic condition programming, based within the Value Based Care (VBC) team. The Project Manager will organize, lead, and manage follow-up for interdisciplinary meetings across clinical stakeholders to formalize clinical programming, policies and define related workstreams to successfully grow and integrate across BMC Primary Care (PC). Oversees all aspects of the project(s), defines project scope, goals and deliverables that support business goals. Sets deadlines, assigns responsibilities, and monitors and summarizes progress of project(s). Prepares reports for upper management regarding status of project(s).

ESSENTIAL RESPONSIBILITIES / DUTIES:

CORE DUTIES:

  • Direct and manage project development from beginning of project lifecycle to end.
  • Define project scope, goals and deliverables that support business goals in collaboration with senior management and stakeholders.
  • Create the project charter and associated communications documents.
  • Effectively communicate project expectations to team members and stakeholders in a timely and clear fashion.
  • Liaise with project stakeholders on an ongoing basis.
  • Estimate the resources and participants needed to achieve project goals.
  • Draft and submit budget proposals, and recommend subsequent budget changes where necessary.

  • Where required, make budget decisions based on basic revenue model and P/L, while maintaining budget to cost projections.
  • Where required, negotiate with other department managers for the acquisition of required personnel from within the company.
  • Set and continually manage project expectations with team members and other stakeholders.
  • Delegate tasks and responsibilities to appropriate personnel.
  • Identify and resolve issues and conflicts within the project team.
  • Identify and manage project dependencies and critical path.
  • Plan and schedule project timelines and milestones using appropriate tools.
  • Track and follow-up on project milestones and deliverables.
  • Develop and deliver progress reports, proposals, requirements documentation, and presentations.
  • Determine the frequency and content of status reports from the project team, analyze results, and troubleshoot problem areas.
  • Proactively manage changes in project scope, identify potential crises, and devise contingency plans.
  • Define project success criteria and disseminate them to involved parties throughout project life cycle.
  • Coach, mentor, motivate and supervise project team members and contractors, and apply soft persuasion skills for them to take positive action and accountability for their assigned work.
  • Build, develop, and grow any business relationships vital to the success of the project.
  • Conduct project post implementation review and create a recommendations report in order to identify successful and unsuccessful project elements.
  • Perform other duties as required for the success of the project.
  • Leads and prepares materials for clinical advisory working groups including Pharmacy, Endocrine, Weight Management/ Nutrition, Integrated Behavioral Health, Ophthalmology, Podiatry, Patient Navigation and Benefit Navigation. Add responsibility for managing workgroup recommendations through adoption and implementation
  • Standardizes meeting design by developing templates and materials to use across the working groups.
  • Works with subject matter experts and VBC team to formalize clinical guidelines, protocolize referrals and standardize reporting.
  • In collaboration with the VBC project team, coordinates meetings and deliverables with BMC validated vendors to promote practice transformation through the implementation of new tools and software.
  • Works with epic IT analysts to organize and launch workflow enhancements to improve PC clinic workflow as it relates to diabetes and hypertension documentation.

JOB REQUIREMENTS

EDUCATION:

Bachelor’s degree in Business Management or field related to the project.  Advanced degree preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Project Management Certification (PMP) from the Project Management Institute (PMI) is highly desired.

EXPERIENCE:

Requires at a minimum 5 years work experience with at least 3 years of direct project management experience, including managing all aspects of process development and execution.

KNOWLEDGE/SKILLS:

  • Knowledge of project management methodologies is required.
  • Strong written and oral communication skills.
  • Strong familiarity with project management software, including Scheduling, Planning, Tracking, Cost Control and Budgeting Tools.
  • Familiarity various software programs and strong computer skills may be required.  Proficiency in one or more of the following products may be required: PowerPoint/Excel/Word/Access, as well as Outlook, and Visio). 
  • Experience at working both independently and in a team-oriented, collaborative environment is essential.
  • Leadership skills and ability to be persuasive, encouraging, and motivating.
  • Ability to conform to shifting priorities, demands and timelines through analytical and problem-solving capabilities.
  • Ability to work cooperatively with other members of the team and staff, ability to handle frequent interruptions and adapt to changes in workload and work schedule; to set priorities and solve problems; to respond quickly to emergency requests.
  • Strong interpersonal skills to elicit cooperation from a wide variety of sources, including upper management, clients, and other departments.
  • Multi-lingual candidates in Haitian Creole, Spanish or Cape Verdean Creole preferred.
