Schedule: Per Diem, As Needed, Non-Benefit Eligible

Primary Summary:

As a Behavioral Health Navigator (BHN), you will work with an interdisciplinary team of medical professionals to determine the appropriate care of all patients who are present to the Emergency Department in behavioral health crisis. BHNs collaborate with outpatient providers, DMH, DCF, families, schools etc. to determine the needs of each patient and to ensure that the patients are receiving the best, most effective care available.

Responsibilities:

  • Performs confidential interviews, both in person and via telemedicine, with patients to assess the needs of patients from referrals directed by Emergency Department physicians.
  • Confers with related health professionals in relation to the need for interdisciplinary information sharing and treatment planning and uncovers barriers to care and develops an effective plan for alleviating these barriers to care for the safety and betterment of the patient.
  • Documents assessments, interventions, and plans related to the patient's confidential medical record in accordance with HIPPA, industry, and compliance standards.
  • Provides education/consultation as needed to other mental health care team members and to health care consumers.
  • Maintains current up-to-date knowledge of regulatory requirements.
  • Provides education and support for clients to engage in their care, improve health literacy, and/or change their health behaviors (e.g., diet, medication adherence, compliance with doctor's visits).
  • Collaborates with the Physician and clinical staff to identify high risk medical/social management patients.
  • Makes referrals to community agencies when appropriate.
  • Identifies potential legal or risk management issues and discusses with their supervisor.
  • Other duties and responsibilities as assigned.

Requirements:

  • MSW or Master’s in Counseling. Doctorate in Psychology or Social Work preferred.
  • 2-3 years of direct diagnostic experience.
  • Licensure at the independent level in the state of Massachusetts; Licensed Psychologists, Licensed Independent Clinical Social Workers, (LICSW) or Licensed Mental Health.
  • Counselors (LMHC) are required.
  • Skilled in the assessment, diagnosis, and disposition of Behavioral Health patients.
  • Knowledgeable about services available to Behavioral Health patients
  • Computer skills are essential.
  • Ability to work in a fast-paced environment.
  • Ability to communicate effectively with internal and external customers with respect to differences in cultures and values.
