Location: Boston, MA

Schedule: 40 hours per week

POSITION SUMMARY:

This position will function as the Project Management Specialist with the Stimulant Treatment and Recovery Team (START). This role combines patient engagement with strong project management skills to oversee initiatives focused on case management, resource connection, community engagement, and group facilitation. The ideal candidate will demonstrate leadership, collaboration, and creativity in building and managing patient-centered programs that improve access to care and strengthen community partnerships.

JOB RESPONSIBILITIES:

Key Responsibilities

Project Management Functions

  • Lead and manage projects that enhance case management services, resource linkage, and patient engagement.
  • Develop project plans, timelines, and deliverables to ensure successful implementation of initiatives.
  • Coordinate cross-functional teams to streamline workflows and reduce barriers to care.
  • Monitor and evaluate project outcomes, preparing regular progress reports for leadership and stakeholders.

Case Management & Resource Connection

  • Provide direct support to patients through needs assessments, service planning, and coordination of care.
  • Build and maintain a robust network of community resources, ensuring timely referrals for housing, healthcare, employment, and recovery supports.
  • Collaborate with internal staff and external partners to improve patient access to critical services.

Community Engagement

  • Represent the program in community meetings, coalitions, and outreach events.
  • Strengthen partnerships with local organizations, healthcare providers, and social service agencies.
  • Develop and implement engagement strategies to enhance patient participation and community awareness of services.

Group Facilitation

  • Facilitate patient groups focused on recovery, life skills, and wellness using evidence-based practices.
  • Foster a supportive, inclusive, and educational group environment.
  • Adapt group content to meet diverse patient needs and stages of recovery.

Documentation & Compliance

  • Maintain accurate and timely clinical documentation in accordance with state, federal, and organizational requirements.
  • Ensure adherence to ethical guidelines, confidentiality standards, and professional codes of conduct.
  • Contribute to audits, quality improvement initiatives, and compliance reviews.

Team Building and Participation

  • Promotes project team efforts and shares sense of purpose so that work is synchronized and concurrent, knowledge is shared, and seamless services are provided.
  • Fosters an environment of camaraderie.
  • Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.
  • Utilizes hospital's RESPECT values as the basis for decision-making and to facilitate the hospital mission.
  • Follows established hospital infection control and safety procedures.

JOB REQUIREMENTS

EDUCATION:

  • Bachelor’s degree in business administration or equivalent experience

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • LADC II, CARC are strongly preferred

EXPERIENCE:

  • Minimum 2 years of experience in substance use counseling, case management, or related field.
  • Demonstrated ability to manage multiple projects simultaneously.
  • Strong facilitation, communication, and organizational skills.

KNOWLEDGE, SKILLS & ABILITIES (KSA):

  • Leadership and teamwork in collaborative settings.
  • Strong interpersonal and motivational skills.
  • Ability to engage diverse patient populations with empathy and cultural humility.
  • Knowledge and experience working with patients who use substances
  • Proficiency in project management tools, electronic health records, and standard office software.
  • Commitment to evidence-based and patient-centered care.
