POSITION SUMMARY:

Under the direction of the Clinical Manager ensures optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Provides drug information to patients and families, physicians, nurses, and other healthcare professional. Integrally involved in education of pharmacists, residents and students. Serves as a resource and drug expert in one of the seven specialty areas. Strives to increase knowledge through research and writing activities. The Clinical Specialist may also be involved, as necessary, in coordinating the acquisition, distribution and control of drug therapy for patients under the care of the pharmacist.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Provides direct patient care in an ambulatory setting with a focus on managing chronic disease states.
  • Performs health histories, medication histories and limited physical examinations as necessary to assess effectiveness of medication therapy. 
  • Evaluates patient(s) response to medication therapy through direct patient involvement and clinical assessment of pertinent subjective and objective findings..
  • Optimizes pharmacotherapy in collaboration with referring physician and in accordance with standard of care guidelines to achieve optimal patient outcomes.
  • Monitors medication therapy and makes modifications as needed based on pertinent laboratory values and/or clinical response..
  • Provides medication and disease state education to patients and caregivers.
  • Assists with management of medication and medication-related problems in clinics. Acts as the drug information resource for the assigned clinic site.
  • Acts as a liaison between the outpatient pharmacies and clinic staff.
  • Assist clinic staff with resolving third party coverage barriers.
  • Documents all interactions with patients in the electronic medical record.
  • Identifies and implements cost-savings and cost-containment programs.
  • Participates in the adverse drug reaction-reporting program and medication error reporting programs.
  • Identifies and implements   quality- improvement initiatives applicable to the ambulatory setting.  
  • Participates in the DUE program by developing DUE’s that are identified according to risk and/or cost, or collecting data of ongoing DUE’s, and presents outcomes and recommendations of DUE’s to Ambulatory P & T committee for endorsement.
  • Precepts pharmacy residents and students..
  • Provides in-service education to pharmacy staff, students, nurses, physicians and other healthcare practitioners, as needed.
  • Participates in medication related research in areas of specialty, when possible.
  • Participates and presents/publishes results of research or noteworthy DUE/Quality Improvement projects at local, regional, and/or national meetings.
  • Participates as a department representative in hospital committees.
  • Attends and participates in department staff development conferences or other activities related to staff development.
  • Maintains his/her professional and specialized competency through continued education.
  • Develops goals each fiscal year based on the institutional scorecard and under the guidance of the Administrative Director of Clinical Affairs.
  • Support 340b program operations as needed. This includes but is not limited to identifying patients that would benefit from the services our outpatient pharmacy provides as well as educating clinic staff on our outpatient pharmacy services,
    • Can include filling, typing or delivering prescriptions on a limited basis

JOB REQUIREMENTS

EDUCATION:

Requires at a minimum a Bachelor’s Degree in Pharmacy and at least five years of pertinent work experience or a Bachelor’s Degree in Pharmacy and post-graduate training, with at least three years of pertinent work experience, or a Doctorate Degree in Pharmacy from an accredited college of pharmacy with post-graduate training or at least two years of relevant work experience.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Work requires licensure to practice pharmacy in the state of Massachusetts and/or eligible for licensure in the state of Massachusetts by the hire date. Applicant can be licensed in another state and pending reciprocation.

KNOWLEDGE AND SKILLS:

  • Deep knowledge of medications, their effects, drug interactions, medical procedures and therapies, diagnosis and disease states required.
  • Effective interpersonal skills to interact appropriately with hospital staff, patients, families/visitors, physicians, colleagues, and others.
  • Ability to multi-task, prioritize essential tasks and meet deadlines
  • An organized work style and ability to keep well written documentation.
  • Excellent verbal and written communication skills. Additional language skills can be a plus.
  • Must possess self-management skills and require little to no supervision. This person must be a self-starter, self-motivating, have a strong work ethic and must have the ability to identify issues or opportunities that require attention and change.
  • Must possess and exhibit process improvement skills and acumen.
  • Strong attention to detail and accuracy.
  • Experience with Microsoft programs (i.e. MS Word, Excel, Outlook) and web browsers.
