POSITION SUMMARY:
The Community Wellness Advocate (CWA) is a member of the StreetCred and Pediatrics team who helps promote health and wellness for families, through integrating economic mobility services particularly focused on maternal-child health equity. CWAs are responsible for assisting with collection of baseline needs assessment data, including consenting families for participation and collecting survey data; providing advocacy and case management services; developing and tracking an economic mobility plan based on identified patient needs; facilitating access to and coordination among economic services and other internal and external resources; monitoring the families’ progress; and problem-solving with patients to both accelerate and enhance access to concrete supports.
CWAs provide clinic-based one-on-one, family, and/or interdisciplinary group support and collaborate with all members of the care team to conduct needs assessments to identify and respond to barriers to the families’ economic well-being in addition to providing economic services as a part of routine well visits. As a member of an innovation model with testing new approaches to build economic well-being, the specific activities of the CWA may evolve over times as the model evolves.
