POSITION SUMMARY:

Performs program assessments and identifies opportunities to improve the Tobacco program. Manage the schedules, patient database, billing, and reimbursement reports. The coordinator will support research opportunities identified by the Medical Director.

Department: Pulmonary Clinic Med Spec

Schedule: Full Time

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Conduct intake and assessment sessions with new and follow up for outpatients. Assists patients in moving through the stages of change by providing supportive counseling in a series of one-on-one coaching sessions. Manage relapse prevention, follow up and after care services.
  • Complete inpatient tobacco consults.
  • Provides support for vaping counseling.
  • Provide resource information and motivational coaching based on patient’s completed assessment and stage of change for smoking cessation.
  • Conduct carbon monoxide and spirometry testing as needed.
  • Develops individualized treatment plans, including appropriate medications for patients in collaboration with the patient’s medical team. Documents the counseling sessions along with the plan of care for each patient.
  • Oversight of the tobacco community health worker position.
  • Develops schedules for inpatient and outpatient counseling and follow up sessions.
  • Maintains patient database on smoking cessation for inpatient and outpatients.
  • Coordinates BMC employee smoking cessation program in collaboration with Human Resources.
  • Maintains and updates tobacco website.
  • Tracks smoking quit rates for outpatients.
  • Develops and runs reports on the patient volume and documentation compliance.
  • Provides education and support for the clinicians on tobacco treatment and other services as needed.
  • Outreach to outside agencies for education, best practice and coordinated care.    
  • Utilizes quality improvement process to identify opportunities to improve the program.

JOB REQUIREMENTS

EDUCATION:

  • Associates degree required in Respiratory Care or related field (behavioral health, addiction treatment, cognitive behavioral therapy)

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • Completion of Tobacco Treatment Specialist Training during onboarding process.

EXPERIENCE:

  • 3-5 years of direct patient care experience with either respiratory diseases and consequences of smoking (e.g. RRT) or patients suffering from addiction and/or mental health (e.g. counselor)
  • 6 months experience in tobacco treatment preferred
  • EPIC experience preferred

KNOWLEDGE AND SKILLS:

  • Knowledge of respiratory and tobacco related diseases, pharmacotherapy.
  • Strong interpersonal skills.
  • Solid understanding of motivational interviewing techniques.
  • The ability to work independently, takes initiative, and handles multiple priorities.
  • Computer proficiency with Microsoft Office, including Excel, PowerPoint and Word.
  • Excellent written and verbal communication skills.
  • Ability to exercise good judgment in dealing with sensitive and confidential information.
  • Requires a knowledge of quality improvement process and a working knowledge of financial systems, budget, and planning.
