JOB SUMMARY:

The Social Worker-LICSW is a clinician responsible for social work services for patients within the BMC Brighton Outpatient Psychiatry Department under the direct supervision of the Chair of Psychiatry and Outpatient Medical Director. This individual is part of the interdisciplinary psych team providing psychotherapy in an outpatient setting. This includes crisis intervention for patients including assessment and treatment of emotional and behavioral problems. Appointments are based on referrals and scheduling consists of telehealth and in person visits.

ESSENTIAL RESPONSIBILITIES / DUTIES:

· Provides individual counseling/therapy to patients

· Provides diagnostic evaluations and psycho-social assessments. ·

Provides referrals and/or follow-up services.

· Collaborate with clinical interdisciplinary team in facilitating continuing care plans.

· Provides counseling for patients, whenever indicated at time of diagnosis, regards adjustment to illness, the emotional impact of diagnosis, and changes in lifestyle.

· Offers continuity by maintaining a supportive relationship to patient and family throughout course of illness.

· On occasions when it is indicated, the social worker will counsel and support patients in bereavement.

· The social worker will provide case management for patients. This will include resource findings, concrete services as indicated and will often involve referrals to community agencies

· Provides documentation for social work activity in patient's record on a regular and timely basis.

· Consults with members of the medical center team in regard to the psycho-social needs of patients.

· Participates where possible in community organizations relevant to the population.

· Makes a psycho-social assessment of as many high-risk clinic patients as time allows.

· Performs other duties as assigned.

Job Requirements

REQUIRED EDUCATION AND EXPERIENCE:

Master’s Degree in Social Work and 0-2 years of experience working with patients and families who are in crisis; or equivalent combination of education and experience.

PREFERRED EDUCATION AND EXPERIENCE

1-2 years of experience with patients with major trauma, infectious disease, and/or emotional upset.

CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED

Current Licensed Independent Clinical Social Work in the state of Massachusetts

KNOWLEDGE, SKILLS & ABILITIES (KSAs)

Ability to work with patients who are in crisis.
