Position Summary:

The Licensed Clinical Social Worker (LCSW) is responsible for the social, emotional, and financial wellbeing of patients on the Senior Behavioral Health Unit. The LCSW will participate in interdisciplinary treatment team planning; is responsible for discharge planning, running treatment groups as assigned, and performing appropriate psychosocial assessments and interventions. The LCSW will assist with referrals to the unit and make referrals to appropriate community agencies as indicated. 

Responsibilities:

  • Develop individualized plans of care with interdisciplinary treatment team.
  • Coordinate discharge/follow-up activities.
  • Discuss the financial implications of placement decisions with patients/families and provide appropriate guidance.
  • Provide individualized psychotherapy, supportive counseling, and group therapy as indicated. Make referrals to community agencies as appropriate.
  • Identify and report any potential legal/risk management issues to the Program Director.
  • Participate in continuing education programs relevant to job function.
  • Document assessments, progress and discharge notes per unit and DMH policy.

Requirements:

  • Education: Master’s Degree in Social Work.
  • Experience: 5 years of relevant experience in a clinical setting with a minimum of 1 year of psychiatric case management experience.
  • Licensure/Certification:
    • Current MA licensure as an LCSW or LICSW
    • BLS certification required.
    • Mandatory De-escalation Education within 30 days of hire.
  • Software/Hardware: Meditech experience preferred; Microsoft Outlook and Word.
  • Strong knowledge of DSM diagnoses, including cognitive impairment.
  • Knowledge of medical terminology.
  • Excellent organizational and case management skills.
  • Ability to multi-task in a fast-paced environment.
  • Ability to work collaboratively and independent of supervision.
