POSITION SUMMARY:

The Population Health Nurse for the BHEAT Program supports the Patient-Centered Outcomes Research Institute (PCORI) research grant and the Remote Blood Pressure Monitoring (RBPM) initiative at GRMDC by serving as the primary clinical point of contact for enrolled patients. Working under the supervision of the Senior Manager of Value-Based Care and Nurse Manager, and in close collaboration with the Site Principal Investigator (PI), project team, and VBC Population Health Nurse, this role ensures that patient data is accurately captured and integrated into the electronic health record (EHR) in alignment with research study protocols. This role will serve as an extension of primary care clinicians, supporting the management of patients with hypertension. Through high-quality care coordination, data accuracy, and patient engagement, the Population Health Nurse advances GRMDC’s mission to improve health outcomes and support innovation in population health and value-based care delivery.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Serves as the primary clinical point of contact for the RBPM initiative at GRMDC.

  • Collaborate closely with the Site PI/Medical Director, Senior Manager of Value-Based Care, Nurse Manager, and project team to ensure alignment with research and clinical objectives.

  • Manage the patient panel enrolled in the RBPM program, including onboarding, monitoring engagement, and coordinating follow-up care.

  • Ensure accurate and timely data flow into the electronic health record (EHR) in accordance with study protocols and research data management requirements.

  • Conduct outreach to patients to verify device functionality, troubleshoot technical or connectivity issues, and assess adherence to monitoring schedules.

  • Provide patient education on blood pressure monitoring, interpretation of readings, and the importance of achieving and maintaining target values.

  • Maintain comprehensive documentation of patient encounters, communications, and study-related data in compliance with clinical and research standards.

  • Participate in team meetings, data reviews, and quality improvement initiatives to support program evaluation and enhance patient outcomes.

  • Performs nursing type duties specific to the department and patient population as directed by supervisors, manager or physician

  • Serve as a liaison between the patient and the care team, including primary care providers and specialists, to ensure seamless communication

JOB REQUIREMENTS

REQUIRED EDUCATION AND EXPERIENCE:

Associates in Nursing and 2-3 years of experience in Ambulatory Office Setting and Adult Primary Care Medicine.

  • Current license in the Commonwealth of Massachusetts to practice as a Licensed Registered Nurse.

  • CPR & BLS certified upon hire (or within first 90 days of employment).

KNOWLEDGE AND SKILLS:

  • Experience in handling patient flow is required.

  • Requires strong computer skills and knowledge of PC applications. Windows environment and Microsoft Office products are required.  Knowledge of the IDX, registration or other comparable registration systems and EMR a plus. 

  • Excellent English communication skills (verbal and written).  Bilingual highly preferred.

  • Excellent professional demeanor. Must be able to work effectively with all levels of staff and management

  • Excellent interpersonal skills. Must be able to show empathy and be courteous and diplomatic with patients.

  • Must frequently work independently, take initiative and handle multiple tasks with conflicting priorities

  • Team player with ability to work cross-organizationally to get issues resolved.

  • Ability to maintain confidentiality.

Compensation Range:

$31.97- $46.39

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer.
