POSITION SUMMARY:

The Care Coordinator is an integral part of the care team and is responsible for the overall case management of the patient’s treatment plan.  Under the direction of the Director of Operations for Stabilization Services, the Care Coordinator works with a multidisciplinary team to ensure that patients have appropriate access to community, CSS, and after care resources.

Department: Brockton Behavioral Health Center

Schedule: 40 Hours (Days)

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Assist patients in the coordination of medication assisted treatment to ensure stabilization treatment and ongoing recovery after care

  • Acts as liaison between patients existing community providers and after care providers to coordinate care upon discharge including accompanying patients to offsite face to face interviews related to securing community base recovery services, primary care services, psychiatric treatment, housing and any other services related to patients treatment plan

  • For Accountable Care Organization (ACO) members enroll patients as appropriate into complex care management

  • Provide patients with resources related to the access for public benefits such as DTA, SSI/SSDI applications

  • Works within a multidisciplinary team to contribute to the overall success of patients recovery by contributing to treatment plans and discharge summaries

  • Acts as an advocate for patients experiencing legal issues by collaborating with probation officers, parole officers, and judicial systems that may impede upon the patients ability to complete treatment

  • Act as a resource to patients by connecting them to financial educational insurances as needed

  • Initiates family meeting for coordination of care as needed

  • Provides after care follow-up phone calls to patients and provide ongoing support for up to 30 days post discharge

  • Provides brief supportive counseling in individual group settings that encourage patients to utilize their therapeutic skills to achieve recovery

  • Performs role according to BSAS, DPH and other local state and federal guidelines, including 42CFR

  • Displays a considerable amount of professional boundaries while working with patients

  • Other duties as assigned

JOB REQUIREMENTS

EDUCATION:

  • Bachelor’s Degree in Social Work or related field.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • Case Management Certificate preferred

EXPERIENCE:

  • At least 1 year working in a Behavioral Health setting.

KNOWLEDGE AND SKILLS:

  • Knowledge of the addiction treatment continuum and related community-based resources

  • Experience in treatment settings for substance use disorders

  • Strong knowledge of dual diagnosis treatment
