The Outpatient Dietitian/ Certified Diabetes Care and Education Specialist is a specialty dietitian who assesses, develops, and coordinates diabetes self-management education for people with type 1, type 2, and gestational diabetes within the scope of the ADA Diabetes Education Program. The position also includes providing medical nutrition therapy for patients referred to the outpatient nutrition program. The dietitian will provide medical nutrition therapy to patients referred for weight management, cardiovascular health, GI disorders, food allergies, and more.
  • Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.
  • Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management.
  • Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, blood glucose monitoring, acute and chronic complications.
  • Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes.
  • Encourage patient adherence to treatment plans and empower them to actively participate in their health
  • Facilitate group education classes on diabetes management
  • Work with diabetes coordinator to design, implement, and conduct diabetes self-management training in accordance to Medicare standards and assist in collecting data for accreditation.  
  • The dietitian plans and provides nutritional care to patients per department policies and procedures by conducting patient assessment and adapting dietary interventions to meet individual patient needs taking into account nutritional status, environmental or social barriers, and cultural diversity utilizing appropriate and proven counseling modalities.   

 

Compensation Range:

$80,500.00- $117,000.00

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
