POSITION SUMMARY:

The Practice Transformation Coach supports practices in their implementation of the TEAM UP model. As a member of the TEAM UP Center’s implementation team, the Practice Transformation Coach will provide training, consultation, coaching, and technical assistance to practices engaged in the TEAM UP Learning Community. This role will require collaboration with clinical training leads and the TEAM UP Center’s evaluation team to ensure all elements of the implementation strategy are well-aligned. The Practice Transformation Coach will support practice leadership to understand and engage in the relevant subject matter and utilize their data to assess implementation progress and inform ongoing improvement. 

We are seeking a candidate with clinical and/or administrative experience working in a community, pediatric primary care, and/or integrated behavioral health practice setting, as well as experience leading data-driven practice transformation or quality improvement efforts. ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Collaborate with healthcare practice partners to facilitate implementation of the TEAM UP model within their organizations

  • Develop professional relationships with organizational leaders, clinical champions, project managers, and practice staff and providers

  • Plan/organize meeting agendas, project trackers, and communications to support implementation and practice transformation efforts. Utilize TEAM UP dashboard, data reports, and other metrics to monitor implementation progress over time and support practice teams in reaching identified certification benchmarks

  • Work with practice teams to develop process maps, swim lane diagrams, standard operating procedures, work plans, and driver diagrams to codify clinical and operations processes. Understand practice team goals for improvement and connect their ideas to the TEAM UP model

  • Introduce and provide coaching on quality improvement and practice transformation tools and methodologies to build capacity within practice teams to lead and sustain improvements

  • Provide training, facilitation, and technical assistance across a variety of activities/events. Support the planning and organization of key TEAM UP Learning Community events and activities

  • Collaborate on the development of training and technical assistance resources and materials, including conducting research into areas of focus/interest and drafting PowerPoint presentations, user guides, and other documents

  • Collaborate with the implementation and evaluation teams to advance knowledge related to integrated behavioral healthcare and the TEAM UP model while contributing to advocacy and dissemination activities. Oversee student interns and/or research assistants in their day-to-day activities, as needed

JOB REQUIREMENTS

EDUCATION AND EXPERIENCE:

  • Master’s degree in the field of medicine, mental health, psychology, social work, or public health; and at least 2 years’ experience working in a supervisory administrative role within a healthcare and/or social service practice setting; and at least 3 years’ experience in quality improvement, project management, health care operations, program development, or equivalent implementation management experience (e.g., public health initiatives, community organizing) required. Or equivalent combination of education and experience.

  • Experience with pediatric and/or behavioral health care required

PREFERRED EDUCATION AND EXPERIENCE:

  • Pediatric primary care and/or integrated behavioral health practice strongly preferred

  • Experience providing training, technical assistance, and consultation

  • Experience utilizing data to inform and monitor practice transformation, implementation, quality improvement, and/or operations efforts

CERTIFICATES, LICENSES, REGISTRATIONS PREFERRED:

  • Certification in quality improvement methodology and/or project management preferred.

KNOWLEDGE, SKILLS & ABILITIES (KSAs):

  • Knowledge of or previous experience with performance improvement methodologies (e.g. Lean Six Sigma, IHI Model for Improvement). Ability to use project management platforms to track work and deliverables

  • Ability to communicate effectively and constructively with excellent oral and written skills. Able to work collaboratively as part of a team, demonstrating trust, respect, and integrity

  • Self-starter, able to take initiative and anticipate organizational needs. Ability to champion innovative models of care delivery and proactively problem solve with partners

  • Objective and able to represent opinions and evidence fairly. Able to manage conflict with diplomacy and tact

  • Able to effectively conduct professional meetings virtually and collaborate/communicate remotely

  • Proficiency in MS Word, Power Point, and Excel is essential

ABOUT THE DEPARTMENT:

The TEAM UP Scaling and Sustainability Center at Boston Medical Center focuses on pediatric behavioral health integration and collaborates with primary care practices to expand access to quality behavioral health care for children and families in multicultural pediatric settings. TEAM UP runs a Learning Community that combines clinical training with practice transformation support to promote implementation of the TEAM UP ModelTM of integrated care. The evaluation arm seeks to advance the evidence base for integrated behavioral health care through research. TEAM UP seeks to ensure prompt access to comprehensive mental, developmental, and behavioral health care for all children.
