POSITION SUMMARY:

The Patient Centered Medical Home Registered Nurse Works collaboratively with physicians, staff and other health care professionals to provide a medical home and care coordination across the health care continuum for all patients within the Primary Care Clinic. Is an integral member of the health care team who works to ensure that safety, best practice and high quality standards of care are maintained across the continuum. Responsible for coordinating a wide range of self-management support and disease registry activities for the clinic's entire patient population.

JOB REQUIREMENTS

EDUCATION:

Diploma or Degree in Nursing required. BS in Nursing preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Must be a licensed Registered Nurse in the Commonwealth of Massachusetts.

EXPERIENCE:


A minimum of 2 years recent experience (within the last 5 years) in ambulatory office adult primary care is required.

KNOWLEDGE AND SKILLS:

  • Ability to work independently and as a member of a multidisciplinary team.
  • Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills.
  • Demonstrates strong analytical, data management and computer skills.
  • Demonstrates strong organizational and time management skills

Compensation Range:

$38.05- $75.33

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, and licensure/certifications directly related to position requirements.
