POSITION SUMMARY:

Boston Medical Center Health System is a ~$7B system whose mission is to provide Exceptional Care without Exception. The Health System includes a tertiary academic medical center a network of community health centers, a health plan and an affiliated accountable care organization.

In 2021 BMC launched the “Health Equity Accelerator” with the purpose of ‘transforming healthcare to deliver health justice and well-being’. The Accelerator, in partnership with Population Health, is developing an innovative multi-disciplinary approach that combines clinical operations, community engagement, health-related social needs programs, and research assets to address racial health inequities. 

The Nurse Practitioner will be a core part of the clinical team caring for patients with cardiometabolic conditions (e.g., diabetes, hypertension, and obesity) in a time-bound program. The Nurse Practitioner will provide treatment, consultation, follow-up care, and medication titration in partnership with a Cardiologist and Endocrinologist.

As an expert professional, the Nurse Practitioner is responsible and accountable for providing high quality and equitable care to patients in this program. The role requires strong communication skills, emotional intelligence, and a commitment to improving health equity.

JOB REQUIREMENTS

EDUCATION:

Master's degree in nursing required. Completion of an accredited educational program for nurse practitioners required.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

Current license to practice as a registered nurse in the Commonwealth of Massachusetts with authorization by the Board of Registration in Nursing to practice in an expanded role.

Registered with the ANCC or other accrediting organization for Nurse Practitioners.

Registered with the State of Massachusetts and DEA for prescription writing privileges.

Current BLS and ACLS Certifications.

EXPERIENCE:

Minimum of three years of current clinical nursing experience and two years experience in adult practice required. Prior experience in related clinical practice preferred.

KNOWLEDGE AND SKILLS:

Clinical care

  • Oversee rapid initiation, titration, and optimization of guideline-directed medical therapy
  • Lead weekly care consultations with the Cardiology and Endocrinology physicians
  • Co-lead community-based shared medical appointments focused on lifestyle modification and self-management along with other multidisciplinary team members (e.g., wellness coach, licensed independent social worker, navigator)
  • Partner with the navigator to support the patient’s home-monitoring, care management and care optimization
  • Collaborate with the community health worker to ensure timely engagement and navigation of patient’s health related social needs

Community engagements

  • Participate in community-based programming to support the mission of the program
  • Demonstrated ability to apply principles of adult learning to provide effective patient and/or family education.
  • Strong interest in social determinants of health and advancing racial health equity.
  • Self-motivated individual with the ability to make independent decisions in the best interest of the patient and population served.
  • Ability to build and manage relationships in a highly complex and changing environment.
  • Demonstrated ability to handle stressful situations in a calm and professional manner.
  • Effective verbal and written communication skills appropriate to the patient populations served.
  • Multilingual skills in languages appropriate to the patient populations served by the medical center preferred (Spanish or Haitian Creole).
  • Computer proficiency inclusive of the ability to access, enter, and interpret computerized data/information required.
  • Physical ability to meet the core job responsibilities in accordance with practice setting demands.
  • All other duties as assigned
