The Living Well at Home Case Manager Supervisor responsible for leadership, supervision and professional development support of LWAH Case Managers Specific responsibilities in these areas include performance management, recruitment, onboarding and participation in educational program planning. The Manager may also provide more intensive hands-on support for individual Case Manager working with challenging cases, to help the Case Managers address social barriers and/or partner more effectively with a community partners. Collaboratively with the LWAH Leadership Team, the Case Manager Supervisor will participate in LWAH program improvement initiatives, bringing unique expertise about social determinants of health and the Case Manager role to program design and implementation strategy. The Case Manager Supervisor also partners with other LWAH and Population Health leaders in building strong relationships with community agencies and resources that support social determinants of health for LWAH patients.

POSITION SUMMARY:

The Living Well At Home (LWAH) Case Manager Supervisor is responsible for leadership, supervision, support and professional development of LWAH staff, whose work is focused on supporting clients who are experiencing homelessness, housing instability and eviction risk. These staff members may include LWAH Case Managers, Housing Stabilization Workers, and Community Wellness Advocates (CWAs), working in any of several locations, on-campus, off-campus and remote.

JOB REQUIREMENTS

EDUCATION:

  • Bachelor’s degree required.

EXPERIENCE:

  • Minimum of 3 years prior experience in housing search and stabilization services, and   eviction prevention services.
  • Expertise in homelessness and housing resources, and eviction prevention.
  • At least one year of leadership experience in similar setting

WORKING CONDITIONS

Generally the work schedule will be Monday through Friday from 9am-5pm. This is an in-person position, and may involve work at multiple program sites in Boston and Cambridge so candidate must have the ability to travel to different program locations and to other community organizations for supervisory and partnership responsibilities as required.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Specific responsibilities include

  • Performance management.
  • Recruiting, onboarding and training of new staff.
  • Providing regular supervision to CWAs, Case Managers and other staff.
  • Expert guidance on challenging cases.
  • Evaluating and assigning client referrals.
  • Participating, along with other LWAH Leaders, in building strong relationships with community agencies and resources that support housing stability and other social determinants of health.
  • Monitoring key performance metrics.
  • Participating in ongoing process improvement.
  • Bringing unique expertise about homelessness, eviction prevention and housing insecurity to program design, strategy and implementation.
  • Assisting with collecting and evaluating program data.
  • Assisting with generating progress reports to funders.
  • Assisting where possible in the preparation of grant proposals.
