POSITION SUMMARY:

Provides social work support services under the supervision of Care Management Leadership for patients admitted to the hospital, focusing on patients' social needs across the continuum of care. Works collaboratively with interdisciplinary team members, including social workers, case managers, medical providers, nursing, and patient financial counselors, to coordinate access to resources, contribute to discharge planning, and assist with targeted patient-centered programs. This role supports various functions including resource navigation, special consults, and program maintenance in alignment with hospital and departmental goals. Receives general supervision and performs duties with some independent judgment within defined protocols and scopes.

Schedule: Monday-Friday 8a-4:30p, One Weekend Per Month

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Collaborates with licensed social workers and the interdisciplinary care team to support patients throughout the continuum of care, including during hospitalization, discharge planning, and post-discharge coordination.

  • Conducts structured screenings and assessments, including those related to geriatrics and substance use disorders, to identify social needs and support patient-centered planning and escalates those requiring mandatory reporting and safety risk cases to the Licensed Clinical Social Worker.

  • Coordinates clothing bank consultations by facilitating emergency clothing access for patients to facilitate discharge, maintaining inventory, ordering supplies, and running usage reports; collaborates with the Development Office to support donation initiatives and storytelling.

  • Coordinates car seat consultations in the Maternal Child Health unit by ensuring car seats are available, engaging with appropriate staff, and supporting patient safety at discharge.

  • Assists with transportation planning, including facilitating PT-1 applications and identifying appropriate transportation resources based on patient need.

  • Provides individualized support to patients who require additional guidance to navigate complex systems, including accessing shelters, food pantries, home health services, and other community-based programs; offers step-by-step assistance and follow-up to ensure successful connection to resources.

  • Supports patients and families with gathering necessary documentation and information for public benefit applications, including MassHealth and long-term care.

  • Provides brief, non-clinical supportive interventions and identifies social barriers to care, escalating any high-risk issues to MSW staff.

  • Accurately documents patient interactions and coordination efforts in the electronic health record (EHR) in accordance with hospital policies and timelines.

  • Participates in staff meetings, supervision, and training sessions to ensure alignment with department protocols and standards of practice

General Duties

  • Provides other administrative and special project assistance as required or assigned

  • Remains knowledgeable of and follows appropriate policies, procedures, and work standards, including but not limited to, policy regarding hours of work, absenteeism, and tardiness

  • Maintains competency in areas of responsibility to ensure quality of care

  • Maintains strict protocols of all confidential or sensitive information

  • Promotes a positive and collaborative work environment supportive of the hospital’s missions and goals

  • Provides a link between Care Management  and other hospital departments

  • Adapts to changing departmental needs, including but not limited to, offering assistance to other team members, adjusting assignments, and other functions

  • Assists in orientation/training of new staff members and volunteers

  • Utilizes hospital's behavioral standards as the basis for decision making and to facilitate the hospital’s mission.

JOB REQUIREMENTS

EDUCATION:

  • Bachelor’s Degree in Social Work (BSW)

EXPERIENCE:

  • One year of relevant experience preferred or equivalent combination of education and experience.

  • One year of experience in a healthcare or community-based setting preferred.

  • Familiarity with public benefits (e.g., MassHealth), long-term care, or resource coordination is a plus.

Compensation Range:

$27.88- $32.33

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, and licensure/certifications directly related to position requirements.
